In case of combination with CYP3A4 inhibitors, the occurrence of docetaxel adverse reactions may
increase, as a result of reduced metabolism. If the concomitant use of a strong CYP3A4 inhibitor (e.g.,
ketoconazole, itraconazole, clarithromycin, indinavir, nefazodone, nelfinavir, ritonavir, saquinavir,
telithromycin and voriconazole) cannot be avoided, a close clinical surveillance is warranted and a
dose-adjustment of docetaxel may be suitable during the treatment with the strong CYP3A4 inhibitor
(see section 4.4). In a pharmacokinetic study with 7 patients, the co-administration of docetaxel with
the strong CYP3A4 inhibitor ketoconazole leads to a significant decrease in docetaxel clearance by
49%.

Potent CYP3A4 inhibitors
Following inhibition of CYP3A4 by co-administration of ketoconazole 200 mg twice daily, Cmax and
AUC of the active metabolite of fesoterodine increased 2.0 and 2.3-fold in CYP2D6 extensive
metabolisers and 2.1 and 2.5-fold in CYP2D6 poor metabolisers, respectively. Therefore, the
maximum dose of fesoterodine should be restricted to 4 mg when used concomitantly with potent
CYP3A4 inhibitors (e.g. atazanavir, clarithromycin, indinavir, itraconazole, ketoconazole, nefazodone,
nelfinavir, ritonavir (and all ritonavir boosted PI-regimens), saquinavir and telithromycin (see
sections 4.2 and 4.4).

In healthy volunteers receiving ketoconazole, a strong CYP3A4 inhibitor, at 200 mg twice daily for
7 days, systemic exposure to lapatinib (100 mg daily) was increased approximately 3.6–fold, and
half-life increased 1.7–fold. Co-administration of Tyverb with strong inhibitors of CYP3A4 (e.g.
ritonavir, saquinavir, telithromycin, ketoconazole, itraconazole, voriconazole, posaconazole,
nefazodone) should be avoided. Co-administration of Tyverb with moderate inhibitors of CYP3A4
should proceed with caution and clinical adverse reactions should be carefully monitored.

Co-administration of temsirolimus 5 mg with ketoconazole, a potent CYP3A4 inhibitor, had no
significant effect on temsirolimus Cmax or AUC; however, sirolimus AUC increased 3.1-fold, and
AUCsum (temsirolimus + sirolimus) increased 2.3-fold compared to temsirolimus alone. The effect on
the unbound concentrations of sirolimus has not been determined, but is expected to be larger than the
effect on whole-blood concentrations due to the saturable binding to red blood cells. The effect may
also be more pronounced at a 25 mg dose. Therefore, substances that are potent inhibitors of
CYP3A4 activity (e.g. nelfinavir, ritonavir, itraconazole, ketoconazole, voriconazole, nefazodone)
increase sirolimus blood concentrations. Concomitant treatment of temsirolimus with these agents
should be avoided (see section 4.4).
