Effects of other substances on bexarotene
No formal studies to evaluate interactions with bexarotene have been conducted. On the basis of the
oxidative metabolism of bexarotene by cytochrome P450 3A4 (CYP3A4), coadministration with other
CYP3A4 substrates such as ketoconazole, itraconazole, protease inhibitors, clarithromycin and
erythromycin may theoretically lead to an increase in plasma bexarotene concentrations. Furthermore,
co-administration with CYP3A4 inducers such as rifampicin, phenytoin, dexamethasone or
phenobarbital may theoretically cause a reduction in plasma bexarotene concentrations.

Systemically available tacrolimus is metabolised via the hepatic Cytochrome P450 3A4 (CYP3A4).
Systemic exposure from topical application of tacrolimus ointment is low (<1.0 ng/ml) and is unlikely
to be affected by concomitant use of substances known to be inhibitors of CYP3A4. However, the
possibility of interactions cannot be ruled out and the concomitant systemic administration of known
CYP3A4 inhibitors (e.g. erythromycin, itraconazole, ketoconazole and diltiazem) in patients with
widespread and/or erythrodermic disease should be done with caution.

Tadalafil is principally metabolised by CYP3A4. A selective inhibitor of CYP3A4, ketoconazole
(200 mg daily), increased tadalafil (10 mg) exposure (AUC) 2-fold and Cmax by 15 %, relative to the
AUC and Cmax values for tadalafil alone. Ketoconazole (400 mg daily) increased tadalafil (20 mg)
exposure (AUC) 4-fold and Cmax by 22 %. Ritonavir, a protease inhibitor (200 mg twice daily), which
is an inhibitor of CYP3A4, CYP2C9, CYP2C19, and CYP2D6, increased tadalafil (20 mg)
exposure (AUC) 2-fold with no change in Cmax. Although specific interactions have not been studied,

Although specific interactions have not been studied,
other protease inhibitors, such as saquinavir, and other CYP3A4 inhibitors, such as erythromycin,
clarithromycin, itraconazole and grapefruit juice should be co-administered with caution as they would
be expected to increase plasma concentrations of tadalafil (see section 4.4).
Consequently the incidence of the adverse reactions listed in section 4.8 might be increased.

Co-administration of erythromycin (500 mg three times a day), a CYP3A4 inhibitor, with vardenafil
(5 mg) resulted in a 4-fold increase in vardenafil AUC and a 3-fold increase in Cmax. Although a specific
interaction study has not been conducted, the co-administration of clarithromycin can be expected to
result in similar effects on vardenafil AUC and Cmax. When used in combination with a moderate
CYP3A4 inhibitor such as erythromycin or clarithromycin, vardenafil dose adjustment might be
necessary (see sections 4.2 and 4.4). Cimetidine (400 mg twice daily), a non-specific cytochrome P450
inhibitor, had no effect on vardenafil AUC and Cmax when co-administered with vardenafil (20 mg) to
healthy volunteers.

Anticholinergic agents may potentially alter the absorption of some concomitantly administered
medicinal products due to anticholinergic effects on gastrointestinal motility. As oxybutynin is
metabolised by cytochrome P 450 isoenzyme CYP3A4, interactions with medicinal products that
inhibit this isoenzyme cannot be ruled out. This should be borne in mind when using azole antifungals
(e.g. ketoconazole) or macrolide antibiotics (e.g. erythromycin) concurrently with oxybutynin.

When co-administered with moderate CYP3A4 inhibitors such as erythromycin, clarithromycin,
telithromycin, fluconazole and grapefruit juice, the recommended starting dose of darifenacin should
be 7.5 mg daily. The dose may be titrated to 15 mg daily to obtain an improved clinical response
provided the dose is well tolerated. Darifenacin AUC24 and Cmax from 30 mg once daily dosing in
subjects who are extensive metabolisers were 95% and 128% higher when erythromycin (moderate
CYP3A4 inhibitor) was co-administered with darifenacin than when darifenacin was taken alone.

QT prolonging medicinal products
- Cardiovascular QT prolonging medicinal products (e.g. quinidine, disopyramide, bepridil, sotalol,
ibutilide, amiodarone).
- Non cardiovascular QT prolonging medicinal products (e.g. pimozide, ziprasidone, sertindole,
mefloquine, halofantrine, pentamidine, cisapride, intravenous erythromycin).
The concomitant use of cardiovascular and non cardiovascular QT prolonging medicinal products with
ivabradine should be avoided since QT prolongation may be exacerbated by heart rate reduction. If the
combination appears necessary, close cardiac monitoring is needed (see section 4.4).
