In case of combination with CYP3A4 inhibitors, the occurrence of docetaxel adverse reactions may
increase, as a result of reduced metabolism. If the concomitant use of a strong CYP3A4 inhibitor (e.g.,
ketoconazole, itraconazole, clarithromycin, indinavir, nefazodone, nelfinavir, ritonavir, saquinavir,
telithromycin and voriconazole) cannot be avoided, a close clinical surveillance is warranted and a
dose-adjustment of docetaxel may be suitable during the treatment with the strong CYP3A4 inhibitor
(see section 4.4). In a pharmacokinetic study with 7 patients, the co-administration of docetaxel with
the strong CYP3A4 inhibitor ketoconazole leads to a significant decrease in docetaxel clearance by
49%.

Sirolimus is extensively metabolised by the CYP3A4 isozyme in the intestinal wall and liver.
Sirolimus is also a substrate for the multidrug efflux pump, P-glycoprotein (P-gp) located in the small
intestine. Therefore, absorption and the subsequent elimination of sirolimus may be influenced by
substances that affect these proteins. Inhibitors of CYP3A4 (such as ketoconazole, voriconazole,
itraconazole, telithromycin, or clarithromycin) decrease the metabolism of sirolimus and increase
sirolimus levels. Inducers of CYP3A4 (such as rifampin or rifabutin) increase the metabolism of
sirolimus and decrease sirolimus levels. Co-administration of sirolimus with strong inhibitors of
CYP3A4 or inducers of CYP3A4 is not recommended (see section 4.4).

Potent CYP3A4 inhibitors
The concomitant use of potent CYP3A4 inhibitors such as azole antifungals (ketoconazole,
itraconazole), macrolide antibiotics (clarithromycin, erythromycin per os, josamycin, telithromycin),
HIV protease inhibitors (nelfinavir, ritonavir) and nefazodone is contraindicated (see section 4.3). The
potent CYP3A4 inhibitors ketoconazole (200 mg once daily) and josamycin (1 g twice daily)
increased ivabradine mean plasma exposure by 7 to 8 fold.
Originalpassage; keine automatisch abgeleitete Schwere, Häufigkeit oder Kausalität. Auch verneinte Befunde und nicht untersuchte Kombinationen bleiben sichtbar.

The most potent of the CYP3A4 inhibitors such as ketoconazole and itraconazole would be expected
to have effects similar to ritonavir (see section 4.3). CYP3A4 inhibitors like clarithromycin,
telithromycin and nefazodone are expected to have an effect in between that of ritonavir and CYP3A4
inhibitors like saquinavir or erythromycin, a seven-fold increase in exposure is assumed. Therefore
dose adjustments are recommended when using CYP3A4 inhibitors (see section 4.2).
