Co-administration of trimethoprim (160 mg twice daily), a moderate CYP2C8 inhibitor, and
repaglinide (a single dose of 0.25 mg) increased the repaglinide AUC, Cmax and t½ (1.6-fold, 1.4-fold
and 1.2-fold respectively) with no statistically significant effects on the blood glucose levels. This lack
of pharmacodynamic effect was observed with a sub-therapeutic dose of repaglinide. Since the safety
profile of this combination has not been established with dosages higher than 0.25 mg for repaglinide
and 320 mg for trimethoprim, the concomitant use of trimethoprim with repaglinide should be
avoided. If concomitant use is necessary, careful monitoring of blood glucose and close clinical
monitoring should be performed (see section 4.4).

Administration of trimethoprim/sulfamethoxazole 160 mg/800 mg results in a 40% increase in
lamivudine exposure, because of the trimethoprim component; the sulfamethoxazole component did
not interact. However, unless the patient has renal impairment, no dosage adjustment of lamivudine is
necessary (see section 4.2). Lamivudine has no effect on the pharmacokinetics of trimethoprim or
sulfamethoxazole. When concomitant administration is warranted, patients should be monitored
clinically. Co-administration of lamivudine with high doses of co-trimoxazole for the treatment of
Pneumocystis jirovecii pneumonia (PCP) and toxoplasmosis should be avoided.

The possibility of interactions with other medicinal products administered concurrently should be
considered, particularly when the main route of elimination is active renal secretion via the organic
cationic transport system e.g. trimethoprim. Other medicinal products (e.g. ranitidine, cimetidine) are
eliminated only in part by this mechanism and were shown not to interact with lamivudine. The
nucleoside analogues (e.g. didanosine) like zidovudine, are not eliminated by this mechanism and are
unlikely to interact with lamivudine.

Lamivudine is cleared renally. Active renal secretion of lamivudine in the urine is mediated through
the OCT2 and multidrug and toxin extrusion transporters (MATE1 and MATE2-K). Trimethoprim (an
inhibitor of these drug transporters) has been shown to increase lamivudine plasma concentrations,
however the resulting increase was not clinically significant (see Table 1). Dolutegravir is an OCT2
and MATE1 inhibitor; however, lamivudine concentrations were similar with or without co-
administration of dolutegravir based on a cross study analysis, indicating that dolutegravir has no
effect on lamivudine exposure in vivo. Lamivudine is also substrate of the hepatic uptake transporter
OCT1. As hepatic elimination plays a minor role in the clearance of lamivudine, drug interactions due
to inhibition of OCT1 are unlikely to be of clinical significance.

As tacrolimus treatment may be associated with hyperkalaemia, or may increase pre-existing
hyperkalaemia, high potassium intake, or potassium-sparing diuretics (e.g. amiloride, triamterene, or
spironolactone) should be avoided (see section 4.4). Care should be taken when tacrolimus is co-
administered with other agents that increase serum potassium, such as trimethoprim and cotrimoxazole
(trimethoprim/sulfamethoxazole), as trimethoprim is known to act as a potassium-sparing diuretic like
amiloride. Close monitoring of serum potassium is recommended.

Lamivudine is cleared renally. Active renal secretion of lamivudine in the urine is mediated through the
OCT2 and multidrug and toxin extrusion transporters (MATE1 and MATE2-K). Trimethoprim (an inhibitor
of these transporters) has been shown to increase lamivudine plasma concentrations, however the resulting
increase was not clinically significant (see Table 1). Dolutegravir is an OCT2 and MATE1 inhibitor;
however, lamivudine concentrations were similar with or without co-administration of dolutegravir based on
a cross study analysis, indicating that dolutegravir has no relevant effect on lamivudine exposure in vivo.
Lamivudine is also substrate of the hepatic uptake transporter OCT1. As hepatic elimination plays a minor
role in the clearance of lamivudine, drug interactions due to inhibition of OCT1 are unlikely to be of clinical
significance.

Certain medicinal products that increase serum potassium (see section 4.4)
Concomitant use of Kerendia with potassium supplements and trimethoprim, or
trimethoprim/sulfamethoxazole is anticipated to increase the risk of hyperkalaemia. Monitoring of
serum potassium is required.
Temporary discontinuation of Kerendia during trimethoprim, or trimethoprim/sulfamethoxazole
treatment may be necessary.
