Medicinal products affected by serum potassium disturbances
Periodic monitoring of serum potassium and ECG is recommended when telmisartan/HCTZ is
administered with medicinal products affected by serum potassium disturbances (e.g. digitalis
glycosides, antiarrhythmics) and the following torsades de pointes inducing medicinal products (which
include some antiarrhythmics), hypokalaemia being a predisposing factor to torsades de pointes.
- class Ia antiarrythmics (e.g. quinidine, hydroquinidine, disopyramide)
- class III antiarrythmics (e.g. amiodarone, sotalol, dofetilide, ibutilide)
- some antipsychotics (e.g. thioridazine, chlorpromazine, levomepromazine, trifluoperazine,
cyamemazine, sulpiride, sultopride, amisulpride, tiapride, pimozide, haloperidol, droperidol)
- others (e.g. bepridil, cisapride, diphemanil, erythromycin IV, halofantrin, mizolastin,
pentamidine, sparfloxacine, terfenadine, vincamine IV.)

• The mode of action suggests that the effects of L-dopa, dopaminergic agonists, and
anticholinergics may be enhanced by concomitant treatment with NMDA-antagonists such as
memantine. The effects of barbiturates and neuroleptics may be reduced. Concomitant
administration of memantine with the antispasmodic agents, dantrolene or baclofen, can modify
their effects and a dose adjustment may be necessary.
• Concomitant use of memantine and amantadine should be avoided, owing to the risk of
pharmacotoxic psychosis. Both compounds are chemically related NMDA-antagonists. The
same may be true for ketamine and dextromethorphan (see also section 4.4). There is one
published case report on a possible risk also for the combination of memantine and phenytoin.
• Other active substances such as cimetidine, ranitidine, procainamide, quinidine, quinine and
nicotine that use the same renal cationic transport system as amantadine may also possibly
interact with memantine leading to a potential risk of increased plasma levels.
• There may be a possibility of reduced serum level of hydrochlorothiazide (HCT) when
memantine is co-administered with HCT or any combination with HCT.
• In post-marketing experience, isolated cases with international normalized ratio (INR) increases
have been reported in patients concomitantly treated with warfarin. Although no causal
relationship has been established, close monitoring of prothrombin time or INR is advisable for
patients concomitantly treated with oral anticoagulants.

ANTINEOPLASTICS AND IMMUNOSUPRESSANTS
Antineoplastics
Apalutamide The mechanism of interaction is CYP3A4 Co-administration with REYATAZ
induction by apalutamide and CYP3A4 (with or without ritonavir) is
inhibition by atazanavir/ritonavir. contraindicated due to the potential
for decreased atazanavir and
ritonavir plasma concentration with
subsequent loss of virologic
response and possible resistance to
the class of protease inhibitors (see
section 4.3). In addition, serum
concentrations of apalutamide may
be increased when coadministered
with atazanavir/ritonavir, resulting
in the potential for serious adverse
events including seizure.
Encorafenib The mechanism of interaction is CYP3A4 Co-administration of encorafenib
inhibition by atazanavir and/or ritonavir. with REYATAZ (with or without
ritonavir) is contraindicated due to
the potential for loss of virologic
response, development of
resistance, increase in encorafenib
plasma concentration and
subsequent risk of serious adverse
events such as QT interval
prolongation (see section 4.3).
Ivosidenib The mechanism of interaction is CYP3A4 Co-administration of ivosidenib
inhibition by atazanavir and/or ritonavir. with REYATAZ (with or without
ritonavir) is contraindicated due to
potential for loss of virologic
response, development of
resistance, increase in ivosidenib
plasma concentration and
subsequent risk of serious adverse
events such as QT interval
prolongation (see section 4.3).
Irinotecan Atazanavir inhibits UGT and may interfere If REYATAZ is co-administered
with the metabolism of irinotecan, with irinotecan, patients should be
resulting in increased irinotecan toxicities. closely monitored for adverse

the risk of ALT elevations due to a significant increase in glecapreir and pibrentasvir plasma

therapeutic area concerning co-administration
Ivosidenib The mechanism of interaction is CYP3A4 Co-administration of
inhibition by atazanavir and/or ritonavir. ivosidenib with REYATAZ
(with or without ritonavir) is
contraindicated due to potential
for loss of virologic response,
development of resistance,
increase in ivosidenib plasma
concentration and subsequent
risk of serious adverse events
such as QT interval
prolongation (see section 4.3).
Irinotecan Atazanavir inhibits UGT and may interfere If REYATAZ is co-
with the metabolism of irinotecan, resulting administered with irinotecan,
in increased irinotecan toxicities. patients should be closely

Antiarrhythmics
Amiodarone, Concentrations of these antiarrhythmics Caution is warranted and
Systemic lidocaine, may be increased when co-administered therapeutic concentration
Quinidine with REYATAZ. The mechanism of monitoring is recommended

In interaction studies in healthy volunteers, Cholestagel had no effect on the bioavailability of digoxin,
metoprolol, quinidine, valproic acid, and warfarin. Cholestagel decreased the Cmax and AUC of
sustained-release verapamil by approximately 31% and 11%, respectively. Since there is a high degree of
variability in the bioavailability of verapamil, the clinical significance of this finding is unclear.

Effect aprepitant on the pharmacokinetics of other active substances
CYP3A4 inhibition
As a moderate inhibitor of CYP3A4, aprepitant (125 mg/80 mg) can increase plasma concentrations of
co-administered active substances that are metabolised through CYP3A4. The total exposure of orally
administered CYP3A4 substrates may increase up to approximately 3-fold during the 3-day treatment
with EMEND; the effect of aprepitant on the plasma concentrations of intravenously administered
CYP3A4 substrates is expected to be smaller. EMEND must not be used concurrently with pimozide,
terfenadine, astemizole, or cisapride (see section 4.3). Inhibition of CYP3A4 by aprepitant could result
in elevated plasma concentrations of these active substances, potentially causing serious or
life-threatening reactions. Caution is advised during concomitant administration of EMEND and orally
administered active substances that are metabolised primarily through CYP3A4 and with a narrow
therapeutic range, such as cyclosporine, tacrolimus, sirolimus, everolimus, alfentanil, diergotamine,
ergotamine, fentanyl, and quinidine (see section 4.4).

An in vitro study shows that zonisamide is a weak inhibitor of P-gp (MDR1) with an IC 50 of 267
µmol/l and there is the theoretical potential for zonisamide to affect the pharmacokinetics of
