Fareston 60 mg tablets · Abschnitt 4.5 · PDF-Seite 4

HOS-085591 · Präparat HOS-083163

An additive effect on QT interval prolongation between Fareston and the following drugs and other medicinal products that may prolong the QTc interval cannot be excluded. This might lead to an increased risk of ventricular arrhythmias, including Torsade de pointes. Therefore co-administration of Fareston with any of the following medicinal products is contraindicated (see also section 4.3): - antiarrhythmics class IA (e.g. quinidine, hydroquinidine, disopyramide) - antiarrhythmics class III (e.g. amiodarone, sotalol, dofetilide, ibutilide) - neuroleptics (e.g. phenothiazines, pimozide, sertindole, haloperidol, sultopride) - certain antimicrobials agents (moxifloxacin, erythromycin IV, pentamidine, antimalarials particularly halofantrine) - certain antihistaminics (terfenadine, astemizole, mizolastine) - others (cisapride, vincamine IV, bepridil, diphemanil).

Co-administration of REYATAZ with grazoprevir-containing products, including elbasvir/grazoprevir fixed-dose combination is contraindicated because of the increase in grazoprevir and elbasvir plasma concentrations and potential for the increase in risk of ALT elevations associated with increased grazoprevir concentrations (see section 4.3). Co-administration of REYATAZ with glecaprevir/pibrentasvir fixed-dose combination is contraindicated because of the potential increase in the risk of ALT elevations due to a significant increase in glecapreir and pibrentasvir plasma concentrations (see section 4.3).

54 Medicinal products by Interaction Recommendations therapeutic area concerning co-administration Ivosidenib The mechanism of interaction is CYP3A4 Co-administration of inhibition by atazanavir and/or ritonavir. ivosidenib with REYATAZ (with or without ritonavir) is contraindicated due to potential for loss of virologic response, development of resistance, increase in ivosidenib plasma concentration and subsequent risk of serious adverse events such as QT interval prolongation (see section 4.3). Irinotecan Atazanavir inhibits UGT and may interfere If REYATAZ is co- with the metabolism of irinotecan, resulting administered with irinotecan, in increased irinotecan toxicities. patients should be closely monitored for adverse events related to irinotecan. Immunosuppressants Cyclosporin Concentrations of these More frequent therapeutic Tacrolimus immunosuppressants may be increased concentration monitoring of Sirolimus when co-administered with REYATAZ due these medicinal products is to CYP3A4 inhibition. recommended until plasma levels have been stabilised. CARDIOVASCULAR AGENTS Antiarrhythmics Amiodarone, Concentrations of these antiarrhythmics Caution is warranted and Systemic lidocaine, may be increased when co-administered therapeutic concentration Quinidine with REYATAZ. The mechanism of monitoring is recommended amiodarone or systemic when available. The lidocaine/atazanavir interaction is CYP3A concomitant use of quinidine is inhibition. Quinidine has a narrow contraindicated (see therapeutic window and is contraindicated section 4.3). due to potential inhibition of CYP3A by REYATAZ.
