In vitro studies using human liver microsomal preparations suggest that CYP 1A2 is the principal
isozyme involved in the initial oxidative metabolism of riluzole. Inhibitors of CYP 1A2 (e.g. caffeine,
diclofenac, diazepam, nicergoline, clomipramine, imipramine, fluvoxamine, phenacetin, theophylline,
amitriptyline and quinolones) could potentially decrease the rate of riluzole elimination, while
inducers of CYP 1A2 (e.g. cigarette smoke, charcoal-broiled food, rifampicin and omeprazole) could
increase the rate of riluzole elimination.

inhibition of the CYP450 enzymes 1A2, 2A6, 2C19, 2D6, 2E1, 3A4/5, and 4A9/11 and was an
inhibitor of CYP2C8 and CYP2C9 as measured using paclitaxel and diclofenac as the probe

not inhibit or induce the metabolism of probe substrates for 1A2 (caffeine), 2C19 (omeprazole), 2C9

Safinamide may transiently inhibit BCRP in vitro. In drug-drug-interaction studies in human, a weak
interaction was observed with rosuvastatin (AUC increase between 1.25 and 2.00 fold) but no
significant interaction was found with diclofenac.
It is recommended to monitor patients when safinamide is taken with medicinal products that are
BCRP substrates (e.g., rosuvastatin, pitavastatin, pravastatin, ciprofloxacin, methotrexate, topotecan,
diclofenac or glyburide) and to refer to their SmPCs to determine if a dose adjustment is needed.

Based on in vitro studies, no clinically relevant interactions between nalmefene, or its metabolites, and
concomitantly administered medicinal products metabolised by the most common CYP450 and UGT
enzymes or membrane transporters are anticipated. Co-administration with medicinal products that are
potent inhibitors of the UGT2B7 enzyme (for example, diclofenac, fluconazole, medroxyprogesterone
acetate, meclofenamic acid) may significantly increase the exposure to nalmefene. This is unlikely to
present a problem with occasional use, but if long-term concurrent treatment with a potent UGT2B7
inhibitor is initiated, a potential for an increase in nalmefene exposure cannot be excluded
(see section 4.4). Conversely, concomitant administration with a UGT inducer (for example,
dexamethasone, phenobarbital, rifampicin, omeprazole) may potentially lead to subtherapeutic
nalmefene plasma concentrations.

In vitro, baricitinib is a substrate for organic anionic transporter (OAT)3, P-glycoprotein (Pgp), breast
cancer resistance protein (BCRP) and multidrug and toxic extrusion protein (MATE)2-K. In a clinical
pharmacology study, dosing of probenecid (an OAT3 inhibitor with strong inhibition potential)
resulted in approximately a 2-fold increase in AUC(0-∞) with no change in tmax or Cmax of baricitinib.
Consequently, in patients taking OAT3 inhibitors with a strong inhibition potential, such as
probenecid, the recommended dose of baricitinib should be reduced by half (see section 4.2). No
clinical pharmacology study has been conducted with OAT3 inhibitors with less inhibition potential.
The prodrug leflunomide rapidly converts to teriflunomide which is a weak OAT3 inhibitor and
therefore may lead to an increase in baricitinib exposure. Since dedicated interaction studies have not
been conducted, caution should be used when leflunomide or teriflunomide are given concomitantly
with baricitinib. Concomitant use of the OAT3 inhibitors ibuprofen and diclofenac may lead to
increased exposure of baricitinib, however their inhibition potential of OAT3 is less compared to
probenecid and thus a clinically relevant interaction is not expected. Coadministration of baricitinib
with ciclosporin (Pgp/BCRP inhibitor) or methotrexate (substrate of several transporters including
OATP1B1, OAT1, OAT3, BCRP, MRP2, MRP3, and MRP4) resulted in no clinically meaningful
effects on baricitinib exposure.
