In vitro studies using human liver microsomal preparations suggest that CYP 1A2 is the principal
isozyme involved in the initial oxidative metabolism of riluzole. Inhibitors of CYP 1A2 (e.g. caffeine,
diclofenac, diazepam, nicergoline, clomipramine, imipramine, fluvoxamine, phenacetin, theophylline,
amitriptyline and quinolones) could potentially decrease the rate of riluzole elimination, while
inducers of CYP 1A2 (e.g. cigarette smoke, charcoal-broiled food, rifampicin and omeprazole) could
increase the rate of riluzole elimination.

Olanzapine does not inhibit the main CYP450 isoenzymes in vitro (e.g. 1A2, 2D6, 2C9, 2C19, 3A4).
Thus no particular interaction is expected as verified through in vivo studies where no inhibition of
metabolism of the following active substances was found: tricyclic antidepressant (representing mostly
CYP2D6 pathway), warfarin (CYP2C9), theophylline (CYP1A2) or diazepam (CYP3A4 and 2C19).

Sedatives/hynoptics
Clorazepate, diazepam, estazolam, Ritonavir co-administration is likely to result in increased plasma concentrations of
flurazepam, oral and parenteral clorazepate, diazepam, estazolam and flurazepam and is therefore contraindicated
midazolam (see section 4.3).
Midazolam is extensively metabolised by CYP3A4. Co-administration with
ritonavir may cause a large increase in the concentration of this benzodiazepine. No
medicinal product interaction study has been performed for the co-administration of
ritonavir with benzodiazepines. Based on data for other CYP3A4 inhibitors, plasma
concentrations of midazolam are expected to be significantly higher when
midazolam is given orally. Therefore, ritonavir should not be co-administered with
orally administered midazolam (see section 4.3), whereas caution should be used
with co-administration of ritonavir and parenteral midazolam. Data from
concomitant use of parenteral midazolam with other protease inhibitors suggest a
possible 3 – 4 fold increase in midazolam plasma levels. If ritonavir is
co-administered with parenteral midazolam, it should be done in an intensive care
unit (ICU) or similar setting which ensures close clinical monitoring and appropriate
medical management in case of respiratory depression and/or prolonged sedation.
Dosage adjustment for midazolam should be considered, especially if more than a
single dose of midazolam is administered.

Plasma exposure of omeprazole (CYP 2C19 substrate) 40 mg once daily was increased by 46%
following administration of valdecoxib 40 mg twice daily for 7 days, while the plasma exposure to
valdecoxib was unaffected. These results indicate that although valdecoxib is not metabolised by
CYP2C19, it may be an inhibitor of this isoenzyme. Therefore, caution should be observed when
administering Dynastat with medicinal products known to be substrates of CYP2C19 (e.g. phenytoin,
diazepam, or imipramine).

In vitro data: Entacapone binds to human albumin binding site II which also binds several other
medicinal products, including diazepam and ibuprofen. According to in vitro studies, significant
displacement is not anticipated at therapeutic concentrations of the medicinal products. Accordingly,
to date there has been no indication of such interactions.

- Effects on other antiepileptic drugs (AEDs)
Inhibition of CYP450 isoenzyme CYP2C19 and CYP3A4 may provoke pharmacokinetic interactions
(inhibition of their hepatic metabolism) with phenobarbital, primidone, phenytoin, carbamazepine,
clobazam (see section 4.2), valproate (see section 4.2), diazepam (enhanced myorelaxation),
ethosuximide, and tiagabine. The consequences are increased plasma levels of these anticonvulsants
with potential risk of overdose. Clinical monitoring of plasma levels of other anticonvulsants when
combined with stiripentol with possible dose adjustments is recommended.
Originalpassage; keine automatisch abgeleitete Schwere, Häufigkeit oder Kausalität. Auch verneinte Befunde und nicht untersuchte Kombinationen bleiben sichtbar.
