In vitro studies using human liver microsomal preparations suggest that CYP 1A2 is the principal
isozyme involved in the initial oxidative metabolism of riluzole. Inhibitors of CYP 1A2 (e.g. caffeine,
diclofenac, diazepam, nicergoline, clomipramine, imipramine, fluvoxamine, phenacetin, theophylline,
amitriptyline and quinolones) could potentially decrease the rate of riluzole elimination, while
inducers of CYP 1A2 (e.g. cigarette smoke, charcoal-broiled food, rifampicin and omeprazole) could
increase the rate of riluzole elimination.

Medicinal products metabolised by CYP2D6: Duloxetine is a moderate inhibitor of CYP2D6. When
duloxetine was administered at a dose of 60 mg twice daily with a single dose of desipramine, a
CYP2D6 substrate, the AUC of desipramine increased 3-fold. The co-administration of duloxetine
(40 mg twice daily) increases steady state AUC of tolterodine (2 mg twice daily) by 71 %, but does
not affect the pharmacokinetics of its active 5-hydroxyl metabolite and no dosage adjustment is
recommended. Caution is advised if YENTREVE is co-administered with medicinal products that are
predominantly metabolised by CYP2D6 (risperidone, tricyclic antidepressants [TCAs] such as

CYP2D6 substrates
After a single 50 mg dose of metoprolol, a CYP2D6 substrate, concomitant administration of repeated
127 mg twice daily doses of eliglustat resulted in a 1.5- and 2.1-fold increase in metoprolol Cmax and
AUC, respectively. Lower doses of medicinal products that are CYP2D6 substrates may be required.
These include certain antidepressants (tricyclic antidepressants, e.g. nortriptyline, amitriptyline,
imipramine, and desipramine), phenothiazines, dextromethorphan and atomoxetine).

The co-administration of duloxetine (40
mg twice daily) increases steady state AUC of tolterodine (2 mg twice daily) by 71 %, but does not
affect the pharmacokinetics of its active 5-hydroxyl metabolite and no dosage adjustment is
recommended. Caution is advised if Duloxetine Lilly is co-administered with medicinal products that
are predominantly metabolised by CYP2D6 (risperidone, tricyclic antidepressants [TCAs] such as
nortriptyline, amitriptyline, and imipramine) particularly if they have a narrow therapeutic index (such
as flecainide, propafenone and metoprolol).
