In vitro studies using human liver microsomal preparations suggest that CYP 1A2 is the principal
isozyme involved in the initial oxidative metabolism of riluzole. Inhibitors of CYP 1A2 (e.g. caffeine,
diclofenac, diazepam, nicergoline, clomipramine, imipramine, fluvoxamine, phenacetin, theophylline,
amitriptyline and quinolones) could potentially decrease the rate of riluzole elimination, while
inducers of CYP 1A2 (e.g. cigarette smoke, charcoal-broiled food, rifampicin and omeprazole) could
increase the rate of riluzole elimination.

Proton pump inhibitors and H2-receptor antagonists
Proton pump inhibitors and H2-receptor antagonists (e.g. omeprazole or ranitidine) may reduce
concentrations for co-administered protease inhibitors. For specific information regarding the impact
of co-administration of acid reducing agents, refer to the SmPC of the co-administered protease
inhibitor. Based on interaction studies with the ritonavir boosted protease inhibitors
(lopinavir/ritonavir, atazanavir), concurrent administration of omeprazole or ranitidine does not
significantly modify ritonavir efficacy as a pharmacokinetic enhancer despite a slight change of
exposure (about 6 - 18%).

Omeprazole 80 mg once daily administered either at the same time as clopidogrel or with 12 hours
between the administrations of the two drugs decreased the exposure of the active metabolite by 45%
(loading dose) and 40% (maintenance dose). The decrease was associated with a 39% (loading dose)

Plasma exposure of omeprazole (CYP 2C19 substrate) 40 mg once daily was increased by 46%
following administration of valdecoxib 40 mg twice daily for 7 days, while the plasma exposure to
valdecoxib was unaffected. These results indicate that although valdecoxib is not metabolised by
CYP2C19, it may be an inhibitor of this isoenzyme. Therefore, caution should be observed when
administering Dynastat with medicinal products known to be substrates of CYP2C19 (e.g. phenytoin,

Effects of other active substances on anagrelide
In vivo interaction studies in humans have demonstrated that digoxin and warfarin do not affect
the pharmacokinetic properties of anagrelide.
CYP1A2 inhibitors
Anagrelide is primarily metabolised by CYP1A2. It is known that CYP1A2 is inhibited by
several medicinal products, including fluvoxamine and enoxacin, and such medicinal products
could theoretically adversely influence the clearance of anagrelide.
CYP1A2 inducers
CYP1A2 inducers (such as omeprazole) could decrease the exposure of anagrelide (see
section 5.2). The consequences on the safety and efficacy profile of anagrelide are not established.
Therefore, clinical and biological monitoring is recommended in patients taking concomitant
CYP1A2 inducers. If needed, anagrelide dose adjustment could be made.

The decrease in AUC, Cmax, and Cmin was
not mitigated when an increased dose of
REYATAZ/ritonavir (400/100 mg once
daily) was temporally separated from
omeprazole by 12 hours. Although not
studied, similar results are expected with
other proton pump inhibitors. This
decrease in atazanavir exposure might
negatively impact the efficacy of
atazanavir. The mechanism of interaction
is decreased solubility of atazanavir as
intra-gastric pH increases with proton
pump inhibitors.
