In an in vitro study, temsirolimus inhibited the transport of P-glycoprotein (P-gp) substrates with an
IC50 value of 2 µM. In vivo, the effect of P-gp inhibition has not been investigated in a clinical
drug-drug interaction study, however, recent preliminary data from a Phase 1 study of combined
lenalidomide (dose of 25 mg) and temsirolimus (dose of 20 mg) seem to support the in vitro findings
and suggest an increased risk of adverse events. Therefore, when temsirolimus is co-administered with
medicinal products which are P-gp substrates (e.g. digoxin, vincristine, colchicine, dabigatran,

P-gp substrates
In vitro data indicate that enzalutamide may be an inhibitor of the efflux transporter P-gp. A mild
inhibitory effect of enzalutamide, at steady-state, on P-gp was observed in a study in patients with
prostate cancer that received a single oral dose of the probe P-gp substrate digoxin before and
concomitantly with enzalutamide (concomitant administration followed at least 55 days of once daily
dosing of 160 mg enzalutamide). The plasma levels of digoxin were measured using a validated liquid
chromatography-tandem mass spectrometry assay. The AUC and Cmax of digoxin increased by 33%
and 17%, respectively. Medicinal products with a narrow therapeutic range that are substrates for P-gp
(e.g. colchicine, dabigatran etexilate, digoxin) should be used with caution when administered
concomitantly with Xtandi and may require dose adjustment to maintain optimal plasma
concentrations.

In vitro, olaparib inhibits the efflux transporter P-gp (IC50=76 µM), therefore it cannot be excluded
that olaparib may cause clinically relevant drug interactions with substrates of P-gp (e.g. simvastatin,
pravastatin, dabigatran, digoxin and colchicine). Appropriate clinical monitoring is recommended for
patients receiving this type of medicinal product concomitantly.

Netupitant/palonosetron capsules are unlikely to interact with medicinal products which are P-gp
substrates. Netupitant is not a substrate for P-gp. When netupitant was administered on Day 8 of a
12-day regimen of digoxin, no changes in digoxin pharmacokinetics were observed.
Inhibition of the efflux transported BCRP and glucuronidation isozyme UGT2B7 by netupitant and
its metabolites is unlikely and, if it occurs, of scarce clinical relevance.
In vitro data shows that netupitant inhibits UGT2B7, the magnitude of such an effect in the clinical
setting is not established. Caution is recommended when netupitant is combined with an oral
substrate of this enzyme (e.g.zidovudine, valproic acid, morphine).
In vitro data suggests that netupitant inhibits the efflux of transporter BCRP. The clinical relevance
of this effect is not established.
In vitro data show that netupitant is a P-gp inhibitor. In a study performed in healthy volunteers,
netupitant does not affect the exposure of digoxin, a P-gp substrate, whereas it increases its Cmax by
1.09 fold [90%CI 0.9-1.31]. It is not excluded that this effect may be more marked, and then clinically
relevant, in cancer patients, notably those having abnormal renal function. Therefore, caution is
recommended when netupitant is combined with digoxin or with other P-gp substrates such as
dabigatran, or colchicine.
