In single-dose studies in healthy volunteers, no interactions were observed between entacapone and
imipramine or between entacapone and moclobemide. Similarly, no interactions between entacapone
and selegiline were observed in repeated-dose studies in parkinsonian patients. However, the
experience of the clinical use of entacapone with several medicinal products, including MAO-A
inhibitors, tricyclic antidepressants, noradrenaline reuptake inhibitors such as desipramine, maprotiline
and venlafaxine, and medicinal products that are metabolised by COMT (e.g. catechol-structured
compounds: rimiterole, isoprenaline, adrenaline, noradrenaline, dopamine, dobutamine, alpha-
methyldopa, apomorphine, and paroxetine) is still limited. Caution should be exercised when these
medicinal products are used concomitantly with entacapone (see also sections 4.3 and 4.4).

In vitro studies indicate that CYP2D6 and CYP3A4 may be minimally involved in paliperidone
metabolism, but there are no indications in vitro nor in vivo that these isozymes play a significant role
in the metabolism of paliperidone. Concomitant administration of oral paliperidone with paroxetine, a
potent CYP2D6 inhibitor, showed no clinically significant effect on the pharmacokinetics of
paliperidone.

CYP2D6 inhibitors
The exposure to vortioxetine increased 2.3-fold for area under the curve (AUC) when vortioxetine
10 mg/day was co-administered with bupropion (a strong CYP2D6 inhibitor 150 mg twice daily) for
14 days in healthy subjects. Co-administration resulted in a higher incidence of adverse reactions
when bupropion was added to vortioxetine than when vortioxetine was added to bupropion.
Depending on individual patient response, a lower dose of vortioxetine may be considered if strong
CYP2D6 inhibitor (e.g., bupropion, quinidine, fluoxetine, paroxetine) is added to vortioxetine
treatment (see section 4.2).

CYP2D6 inhibitors
Co-administration of a 2 mg single oral dose of brexpiprazole with quinidine (324 mg/day for 7 days),
a potent inhibitor of CYP2D6, increased the AUC of brexpiprazole by 94 % and did not change the
Cmax. Based on results of interaction studies, dose adjustment of brexpiprazole to half the dose is
recommended when administered concomitantly with strong CYP2D6 inhibitors (quinidine,
paroxetine, and fluoxetine).

Coadministration of a single 0.35 mg/kg dose of fenfluramine with paroxetine (a strong CYP2D6
inhibitor) at steady state (30 mg once daily) in healthy volunteers increased the AUC 0-t of
fenfluramine by a ratio of 1.8-fold and the C max by a ratio of 1.1-fold, and decreased the AUC 0-t of
norfenfluramine by a ratio of 1.2-fold and the C max by a ratio of 1.3-fold, as compared to fenfluramine
administered alone.

Quinidine and other CYP2D6 inhibitors
In a clinical trial in healthy subjects, a strong inhibitor of CYP2D6 (quinidine) increased aripiprazole AUC
by 107%, while Cmax was unchanged. The AUC and Cmax of dehydro-aripiprazole, the active metabolite,
decreased by 32% and 47%, respectively. Aripiprazole dose should be reduced to approximately one-half of
its prescribed dose when concomitant administration of aripiprazole with quinidine occurs. Other strong
inhibitors of CYP2D6, such as fluoxetine and paroxetine, may be expected to have similar effects and similar
dose reductions should therefore be applied.
