Effect on BCRP (Breast Cancer Resistance Protein) and/or organic anion transporting polypeptide B1
and B3 (OATP1B1/B3) substrates
There was an increase in mean rosuvastatin Cmax and AUC (2.65- and 2.51-fold, respectively),
following repeated doses of A771726. However, there was no apparent impact of this increase in
plasma rosuvastatin exposure on the HMG-CoA reductase activity. If used together, the dose of
rosuvastatin should not exceed 10 mg once daily. For other substrates of BCRP (e.g. methotrexate,
topotecan, sulfasalazine, daunorubicin, doxorubicin) and the OATP family especially HMG-CoA
reductase inhibitors (e.g. simvastatin, atorvastatin, pravastatin, methotrexate, nateglinide, repaglinide,
rifampicin) concomitant administration should also be undertaken with caution. Patients should be
closely monitored for signs and symptoms of excessive exposure to the medicinal products and
reduction of the dose of these medicinal products should be considered.

Rosuvastatin (BCRP substrate)
Ticagrelor has been shown to increase rosuvastatin Cmax by approximately 2.5-fold and AUC by
approximately 2.4-fold, which may result in increased risk of myopathy, including rhabdomyolysis.
Consideration should be given to the benefits of prevention of major adverse cardiovascular events by
use of rosuvastatin versus the risks with increased rosuvastatin plasma concentrations.

The effects of vemurafenib on medicinal products that are substrates of BCRP are unknown. It cannot
be excluded that vemurafenib may increase the exposure of medicines transported by BCRP (e.g.

Many anticancer medicinal products are substrates of BCRP and therefore there is a theoretical risk
for an interaction with vemurafenib.

by 50% is recommended for coadministration with teriflunomide. For other substrates of BCRP (e.g.,
methotrexate, topotecan, sulfasalazine, daunorubicin, doxorubicin) and the OATP family especially
HMG-Co reductase inhibitors (e.g., simvastatin, atorvastatin, pravastatin, methotrexate, nateglinide,
repaglinide, rifampicin) concomitant administration of teriflunomide should also be undertaken with
caution. Patients should be closely monitored for signs and symptoms of excessive exposure to the
medicinal products and reduction of the dose of these medicinal products should be considered.

Breast cancer resistance protein (BCRP) and P-glycoprotein substrates
Administration of regorafenib (160 mg for 14 days) prior to administration of a single dose of
rosuvastatin (5 mg), a BCRP substrate, resulted in a 3.8-fold increase in mean exposure (AUC) of
rosuvastatin and a 4.6-fold increase in Cmax.

HMG-CoA reductase inhibitors (“statins”)
Lomitapide increases plasma concentrations of statins. When lomitapide 60 mg was administered to
steady state prior to simvastatin 40 mg, simvastatin acid AUC and Cmax increased 68% and 57%,
respectively. When lomitapide 60 mg was administered to steady state prior to atorvastatin 20 mg,
atorvastatin acid AUC and Cmax increased 52% and 63%, respectively. When lomitapide 60 mg was
administered to steady state prior to rosuvastatin 20 mg, rosuvastatin time to maximum concentration
(Tmax) increased from 1 to 4 hours, AUC was increased 32%, and its Cmax was unchanged. The risk of
myopathy with simvastatin is dose related. Use of Lojuxta is contraindicated in patients treated with
high doses of simvastatin (> 40 mg) (see sections 4.3 and 4.4).

Effects on specific enzymes and transporters
In vitro studies indicate that vismodegib has the potential to act as an inhibitor of breast cancer
resistance protein (BCRP). In vivo interaction data is not available. It may not be excluded that
vismodegib may give rise to increased exposure of medicinal products transported by this protein,
such as rosuvastatin, topotecan, and sulfasalazin. Concomitant administration should be performed
with caution and a dose adjustment may be necessary.
