Methylthioninium chloride Proveblue 5 mg/ml solution for injection · Abschnitt 4.5 · PDF-Seite 5

HOS-213653 · Präparat HOS-155158

Methylthioninium chloride should be avoided in patients receiving medicinal products that enhance serotonergic transmission because of the potential for serious CNS reactions, including potentially fatal serotonin syndrome. These include SSRIs (selective serotonin reuptake inhibitors), bupropion, buspirone, clomipramine, mirtazapine, and venlafaxine. Opioids, for example, tramadol, fentanyl, pethidine, and dextromethorphan, may also increase the risk of developing serotonin syndrome, when used in combination with methylthioninium chloride. If the intravenous use of methylthioninium chloride cannot be avoided in patients treated with serotonergic medicinal products, the lowest possible dose should be chosen and the patient observed closely for central nervous system (CNS) effects for up to 4 hours after administration (see sections 4.4 and 4.8).

CYP3A substrates Tucatinib is a strong CYP3A inhibitor. A clinical drug interaction study found that co-administration of tucatinib with midazolam (a sensitive CYP3A substrate) resulted in an increase in midazolam concentrations (3.0-fold Cmax (90% CI: 2.6, 3.4) and 5.7-fold AUC (90% CI: 5.0, 6.5)). Co-administration of tucatinib with sensitive CYP3A substrates such as alfentanil, avanafil, buspirone, darifenacin, darunavir, ebastine, everolimus, ibrutinib, lomitapide, lovastatin, midazolam, naloxegol, saquinavir, simvastatin, sirolimus, tacrolimus, tipranavir, triazolam, and vardenafil may increase their systemic exposures which may increase the toxicity associated with a CYP3A substrate. Concomitant use of tucatinib with CYP3A substrates, when minimal concentration changes may lead to serious or life-threatening toxicities, should be avoided. If concomitant use is unavoidable, the CYP3A substrate dosage should be decreased in accordance with the concomitant medicinal product SmPC.

CYP3A4/5 substrates In a clinical study, concomitant use of camizestrant increased midazolam, a sensitive CYP3A4/5 substrate, geometric mean Cmax by 1.50-fold and AUC by 1.99-fold respectively, therefore, camizestrant is a weak inhibitor of CYP3A4/5. Camizestrant should be used with caution when co- administered with sensitive CYP3A4/5 substrates (including but not limited to: budesonide, buspirone, dronedarone, ivabradine, lurasidone, midazolam, quetiapine, sildenafil and simvastatin). Narrow therapeutic index CYP3A4/5 substrates (e.g. alfentanil and fentanyl) may require dose adjustment and should be monitored closely.
