CYP2D6 substrates
In a clinical study, naltrexone/bupropion (32 mg naltrexone hydrochloride /360 mg bupropion
hydrochloride daily) was co-administered with a 50 mg dose of metoprolol (a CYP2D6 substrate).
Naltrexone/bupropion increased metoprolol AUC and Cmax by approximately 4- and 2-fold,
respectively, relative to metoprolol alone. Similar clinical drug interactions resulting in increased

pharmacokinetic exposure of CYP2D6 substrates have also been observed with bupropion as a single
medicinal product with desipramine and venlafaxine.

Medicinal products affecting pitolisant metabolism
- CYP2D6 inhibitors
CYP2D6 inhibitors will most likely have an effect on the pharmacokinetics of pitolisant in subjects
that are CYP2D6 intermediate, extensive metabolizers or ultra-rapid metabolizers and taking no
CYP3A inducers, but not in subjects that are CYP2D6 poor metabolizers or intermediate, extensive
metabolizers or CYP2D6 ultra-rapid metabolizers and taking CYP3A inducers. A dosage adjustment
during the combination could eventually be considered depending on individual response and
tolerance.
Co-administration of pitolisant with paroxetine alone or combined with CY3A4 inhibitors
significantly increases to the same extent pitolisant mean Cmax and AUC0—72h ratio about 1.5-fold and
2-fold, respectively. Given the 2-fold increase of pitolisant exposure, its coadministration with
CYP2D6 inhibitors (e.g. paroxetine, fluoxetine, venlafaxine, duloxetine, bupropion, quinidine,
terbinafine, cinacalcet) alone or combined with a CYP3A4 inhibitors (itraconazole, ketonazole) should
be done with caution.

CYP2D6, particularly with medicinal products that have a narrow therapeutic index. Dose reduction of
medicinal products with a narrow therapeutic index that are metabolised by CYP2D6 should be

Effects of niraparib or abiraterone acetate on other medicinal products
CYP2D6 substrates
Abiraterone is an inhibitor of CYP2D6. In a clinical study to determine the effects of abiraterone
acetate plus prednisone (AAP) on a single dose of the CYP2D6 substrate dextromethorphan, the
systemic exposure (AUC) of dextromethorphan was increased approximately 2.9-fold. The AUC24 for
dextrorphan, the active metabolite of dextromethorphan, increased approximately 33%. Dose
reduction of medicinal products with a narrow therapeutic index that are metabolised by CYP2D6
should be considered. Examples of medicinal products metabolised by CYP2D6 include metoprolol,
propranolol, desipramine, venlafaxine, haloperidol, risperidone, propafenone, flecainide, codeine,
oxycodone and tramadol.

Camizestrant in combination with ribociclib has been studied clinically, a non-fatal Torsades de
Pointes was observed in a patient with bradycardia and QTc prolongation for camizestrant in
combination with ribociclib. It is recommended to monitor patients clinically and adjust doses as
specified in the approved ribociclib SmPC. Co-administration of other medicinal products known to
prolong the QT interval and/or have a known risk of Torsades de Pointes (e.g. ciprofloxacin,
levofloxacin, ondansetron, escitalopram, venlafaxine, fluconazole, chloroquine, halofantrine,
clarithromycin, azithromycin, haloperidol, methadone, moxifloxacin, bepridil, and pimozide) should
be avoided. If concomitant use cannot be avoided, monitor ECGs as clinically indicated (see section
4.2).
