Increased systemic exposure to didanosine may increase didanosine related adverse reactions. Rarely, pancreatitis and lactic acidosis, sometimes fatal, have been reported. Co-administration of tenofovir disoproxil and didanosine at a dose of 400 mg daily has been associated with a significant decrease in CD4 cell count, possibly due to an intracellular interaction increasing phosphorylated (i.e. active) didanosine. A decreased dosage of 250 mg didanosine co-administered with tenofovir disoproxil therapy has been associated with reports of high rates of virological failure within several tested combinations for the treatment of HIV-1 infection. Adefovir dipivoxil AUC: ↔ Tenofovir disoproxil should Cmax: ↔ not be administered concurrently with adefovir dipivoxil (see section 4.4).

Likewise, tafamidis inhibits the uptake transporters OAT1 and OAT3 (organic anion transporters) with IC50=2.9 µM and IC50=2.36 µM, respectively, and may cause drug-drug interactions at clinically relevant concentrations with substrates of these transporters (e.g. non-steroidal anti-inflammatory drugs, bumetanide, furosemide, lamivudine, methotrexate, oseltamivir, tenofovir, ganciclovir, adefovir, cidofovir, zidovudine, zalcitabine). Based on in vitro data, the maximal predicted changes in AUC of OAT1 and OAT3 substrates were determined to be less than 1.25 for the tafamidis meglumine 20 mg dose, therefore, inhibition of OAT1 or OAT3 transporters by tafamidis is not expected to result in clinically significant interactions.

OAT3 substrates Leniolisib is an OAT3 inhibitor and may increase systemic exposure to OAT3 substrates (e.g., adefovir, baricitinib, bumetanide, cefaclor, ceftizoxime, ciprofloxacin, famotidine, furosemide, methotrexate, oseltamivir carboxylate, benzylpenicillin [penicillin G], tenofovir). When co- administered, leniolisib increased furosemide exposure 1.4-fold. Avoid concomitant use of leniolisib with medicinal products that are OAT3 substrates with a narrow therapeutic index (e.g., methotrexate).
