Other statins and fibrates
Gemfibrozil increases the AUC of simvastatin acid by 1.9-fold, possibly due to inhibition of the
glucuronidation pathway. The risk of myopathy and rhabdomyolysis is significantly increased by
concomitant use of gemfibrozil with simvastatin. The risk of rhabdomyolysis is also increased in
patients concomitantly receiving other fibrates or statins. Therefore, the co-administration of Cholib
with gemfibrozil, other fibrates, or statins is contraindicated (see section 4.3).

CYP2C8 inhibitors
CYP2C8 plays an important role in the elimination of enzalutamide and in the formation of its active
metabolite. Following oral administration of the strong CYP2C8 inhibitor gemfibrozil (600 mg twice
daily) to healthy male subjects, the AUC of enzalutamide increased by 326% while Cmax of
enzalutamide decreased by 18%. For the sum of unbound enzalutamide plus the unbound active
metabolite, the AUC increased by 77% while Cmax decreased by 19%. Strong inhibitors (e.g.
gemfibrozil) of CYP2C8 are to be avoided or used with caution during enzalutamide treatment. If
patients must be co-administered a strong CYP2C8 inhibitor, the dose of enzalutamide should be
reduced to 80 mg once daily (see section 4.2).

In the presence of 600 mg gemfibrozil, twice a day, a strong inhibitor of CYP2C8, exposure to
selexipag increased approximately 2-fold, whereas exposure to the active metabolite, the major
contributor to efficacy, increased approximately 11-fold. Concomitant administration of Uptravi with

Strong CYP3A4 inhibitors and UGT1A1 inhibitors
Patients receiving concomitant non-liposomal irinotecan and ketoconazole, a CYP3A4 and
UGT1A1 inhibitor, have increased SN-38 exposure by 109%. Therefore, co-administration of
ONIVYDE pegylated liposomal with other inhibitors of CYP3A4 (e.g. grapefruit juice,
clarithromycin, indinavir, itraconazole, lopinavir, nefazodone, nelfinavir, ritonavir, saquinavir,
telaprevir, voriconazole) may increase systemic exposure of ONIVYDE pegylated liposomal. Based
on the drug interaction of non-liposomal irinotecan and ketoconazole, co-administration of ONIVYDE
pegylated liposomal with other inhibitors of UGT1A1 (e.g. atazanavir, gemfibrozil, indinavir,
regorafenib) may also increase systemic exposure of ONIVYDE pegylated liposomal.

CYP2C8 plays a role in the elimination of apalutamide and in the formation of its active metabolite. In
a drug-drug interaction study, the Cmax of apalutamide decreased by 21% while AUC increased by
68% following co-administration of apalutamide 240 mg single dose with gemfibrozil (strong
CYP2C8 inhibitor). For the active moieties (sum of apalutamide plus the potency adjusted active
metabolite), Cmax decreased by 21% while AUC increased by 45%. No initial dose adjustment is
necessary when Erleada is co-administered with a strong inhibitor of CYP2C8 (e.g., gemfibrozil,
clopidogrel) however, a reduction of the Erleada dose based on tolerability should be considered (see
section 4.2). Mild or moderate inhibitors of CYP2C8 are not expected to affect the exposure of

Inhibitors of OATP1B1 or 3
Co-administration of letermovir with medicinal products that are inhibitors of OATP1B1/3
transporters may result in increased letermovir plasma concentrations. If letermovir is co-administered
with cyclosporine (a potent OATP1B1/3 inhibitor), the recommended dose of letermovir is 240 mg
once daily in adult and paediatric patients weighing at least 30 kg (see Table 1 and sections 4.2 and
5.2). If oral letermovir is co-administered with cyclosporine in paediatric patients weighing less than
30 kg, the dose should be decreased (see sections 4.2 and 5.2). Caution is advised if other OATP1B1/3
inhibitors are added to letermovir combined with cyclosporine.

Inhibitors of OATP1B1 or 3
Co-administration of letermovir with medicinal products that are inhibitors of OATP1B1/3
transporters may result in increased letermovir plasma concentrations. If letermovir is co-administered
with cyclosporine (a potent OATP1B1/3 inhibitor), the recommended dose of letermovir is 240 mg
once daily in adult and paediatric patients weighing at least 30 kg (see Table 2 and sections 4.2 and
5.2). If intravenous letermovir is co-administered with cyclosporine in paediatric patients weighing
less than 30 kg, dose adjustment is not required (see Table 2 and sections 4.2 and 5.2). Caution is
advised if other OATP1B1/3 inhibitors are added to letermovir combined with cyclosporine.

The coadministration of gemfibrozil (a strong inhibitor of CYP2C8) 600 mg twice daily at steady state
and a single dose of ozanimod 0.46 mg increased exposure (AUC) of the major active metabolites by
approximately 47% to 69%. Caution should be exercised for concomitant use of ozanimod with strong
CYP2C8 inhibitors (e.g. gemfibrozil, clopidogrel).

Modifiers of CYP2C8 or UGT1A9 activity
Roxadustat is a substrate of CYP2C8 and UGT1A9. Co-administration of roxadustat with gemfibrozil
(CYP2C8 and OATP1B1inhibitor) or probenecid (UGT and OAT1/OAT3 inhibitor) in healthy
subjects increased roxadustat AUC by 2.3-fold and Cmax by 1.4-fold. Hb levels are to be monitored
when initiating or discontinuing concomitant treatment with gemfibrozil, probenecid, other strong
inhibitors or inducers of CYP2C8 or other strong inhibitors of UGT1A9. The dose of roxadustat is to
be adjusted following dose adjustment rules (see Table 2) based on Hb monitoring.

CYP2C8 inhibitors
A clinical drug interaction study found that co-administration of a single dose of 300 mg tucatinib with
gemfibrozil (a strong CYP2C8 inhibitor) resulted in an increase in tucatinib concentrations (1.6-fold
Cmax (90% CI: 1.5, 1.8) and 3.0-fold AUC (90% CI: 2.7, 3.5)). Co-administration of tucatinib with
strong CYP2C8 inhibitors such as gemfibrozil should be avoided as this may result in increased risk of
tucatinib toxicity (see section 4.4).
