Based on the results from the population pharmacokinetic analysis of patients with partial-onset
seizures and patients with primary generalised tonic-clonic seizures the total clearance of Fycompa
was increased when co-administered with carbamazepine (3-fold), and phenytoin or oxcarbazepine
(2-fold), which are known inducers of enzymes of metabolism (see section 5.2). This effect should be
taken into account and managed when adding or withdrawing these anti-epileptic drugs from a
patient’s treatment regimen. Clonazepam, levetiracetam, phenobarbital, topiramate, zonisamide,
clobazam, lamotrigine and valproic acid did not affect to a clinically relevant manner the clearance of
Fycompa.

Netupitant/palonosetron capsules are unlikely to interact with medicinal products which are P-gp
substrates. Netupitant is not a substrate for P-gp. When netupitant was administered on Day 8 of a
12-day regimen of digoxin, no changes in digoxin pharmacokinetics were observed.
Inhibition of the efflux transported BCRP and glucuronidation isozyme UGT2B7 by netupitant and
its metabolites is unlikely and, if it occurs, of scarce clinical relevance.
In vitro data shows that netupitant inhibits UGT2B7, the magnitude of such an effect in the clinical
setting is not established. Caution is recommended when netupitant is combined with an oral
substrate of this enzyme (e.g.zidovudine, valproic acid, morphine).
In vitro data suggests that netupitant inhibits the efflux of transporter BCRP. The clinical relevance
of this effect is not established.
In vitro data show that netupitant is a P-gp inhibitor. In a study performed in healthy volunteers,
netupitant does not affect the exposure of digoxin, a P-gp substrate, whereas it increases its Cmax by
1.09 fold [90%CI 0.9-1.31]. It is not excluded that this effect may be more marked, and then clinically
relevant, in cancer patients, notably those having abnormal renal function. Therefore, caution is
recommended when netupitant is combined with digoxin or with other P-gp substrates such as
dabigatran, or colchicine.

Co-administration of guanfacine and valproic acid can result in increased concentrations of valproic
acid. The mechanism of this interaction is unknown, although both guanfacine and valproic acid are
metabolised by glucuronidation, possibly resulting in competitive inhibition. When guanfacine is
co-administered with valproic acid, patients should be monitored for potential additive central nervous
system (CNS) effects and consideration should be given to the monitoring of serum valproic acid
concentrations. Adjustments in the dose of valproic acid and guanfacine may be indicated when
co-administered.

Inhibitors of UGT1A3 and UGT2B7
The effect of strong inhibitors of UGT1A3 and UGT2B7 (valproic acid, probenecid, and fluconazole)
on the exposure to selexipag and its active metabolite has not been studied. Caution is required when
administering these medicinal products concomitantly with Uptravi. A potential pharmacokinetic
interaction with strong inhibitors of UGT1A3 and UGT2B7 cannot be excluded.

In vivo studies and population pharmacokinetic analysis
Accordingly, in in vivo studies no clinically relevant pharmacokinetic interactions were observed
between pregabalin and phenytoin, carbamazepine, valproic acid, lamotrigine, gabapentin, lorazepam,
oxycodone or ethanol. Population pharmacokinetic analysis indicated that oral antidiabetics, diuretics,
insulin, phenobarbital, tiagabine and topiramate had no clinically significant effect on pregabalin
clearance.

Effect of fenfluramine on other medicinal products
Co-administration of a single 0.7 mg/kg dose of fenfluramine, with a single dose of a stiripentol,
clobazam, and valproic acid combination, did not affect the pharmacokinetics of stiripentol, nor the
pharmacokinetics of clobazam or its Ndesmethyl-metabolite norclobazam, nor the pharmacokinetics
of valproic acid, as compared to the stiripentol, clobazam, and valproic acid combination alone.
