Likewise, tafamidis inhibits the uptake transporters OAT1 and OAT3 (organic anion transporters) with
IC50=2.9 µM and IC50=2.36 µM, respectively, and may cause drug-drug interactions at clinically
relevant concentrations with substrates of these transporters (e.g. non-steroidal anti-inflammatory
drugs, bumetanide, furosemide, lamivudine, methotrexate, oseltamivir, tenofovir, ganciclovir,
adefovir, cidofovir, zidovudine, zalcitabine). Based on in vitro data, the maximal predicted changes in
AUC of OAT1 and OAT3 substrates were determined to be less than 1.25 for the tafamidis 61 mg
dose, therefore, inhibition of OAT1 or OAT3 transporters by tafamidis is not expected to result in
clinically significant interactions.

Effect of teriflunomide on organic anion transporter 3 (OAT3) substrates
There was an increase in mean cefaclor Cmax and AUC (1.43- and 1.54-fold, respectively), following
repeated doses of teriflunomide, suggesting that teriflunomide is an inhibitor of OAT3 in vivo.
Therefore, when teriflunomide is coadministered with substrates of OAT3, such as cefaclor,
benzylpenicillin, ciprofloxacin, indomethacin, ketoprofen, furosemide, cimetidine, methotrexate,
zidovudine, caution is recommended.

Tazobactam is a substrate for OAT1 and OAT3. In vitro, tazobactam inhibited human OAT1 and
OAT3 transporters with IC50 values of 118 and 147 mcg/mL, respectively. Co-administration of
ceftolozane/tazobactam with OAT1 and OAT3 substrate furosemide in a clinical study did not
significantly increase furosemide plasma exposures (geometric mean ratios of 0.83 and 0.87 for Cmax
and AUC, respectively). However, active substances that inhibit OAT1 or OAT3 (e.g., probenecid)
may increase tazobactam plasma concentrations.

As sodium zirconium cyclosilicate is not absorbed or metabolised by the body, and does not
meaningfully bind other medicinal products, there are limited effects on other medicinal products.
Sodium zirconium cyclosilicate can transiently increase gastric pH by absorbing hydrogen ions and
can lead to changes in solubility and absorption kinetics for co-administered medicinal products with
pH-dependent bioavailability. In a clinical drug-drug interaction study conducted in healthy subjects
co-administration of sodium zirconium cyclosilicate with amlodipine, clopidogrel, atorvastatin,
furosemide, glipizide, warfarin, losartan or levothyroxine did not result in clinically meaningful drug-
drug interactions. Consistent with co-administration of dabigatran with other gastric acid modifiers,
dabigatran Cmax and AUC values were approximately 40% lower when co-administered with sodium
zirconium cyclosilicate. No dose adjustments or separation of time of dosing are required for any of
these medicinal products. However, sodium zirconium cyclosilicate should be administered at least
2 hours before or 2 hours after oral medicinal products with clinically meaningful gastric pH
dependent bioavailability.

Furosemide
Co-administration of sacubitril/valsartan and furosemide had no effect on the pharmacokinetics of
sacubitril/valsartan but reduced Cmax and AUC of furosemide by 50% and 28%, respectively. While
there was no relevant change in urine volume, the urinary excretion of sodium was reduced within
4 hours and 24 hours after co-administration. The average daily dose of furosemide was unchanged
from baseline until the end of the PARADIGM-HF study in patients treated with sacubitril/valsartan.
Originalpassage; keine automatisch abgeleitete Schwere, Häufigkeit oder Kausalität. Auch verneinte Befunde und nicht untersuchte Kombinationen bleiben sichtbar.

Transporter substrates
In vivo, encorafenib is an inhibitor of OATP1B1, OATP1B3 and/or BCRP. Coadministration of
encorafenib with OATP1B1, OATP1B3 or BCRP substrates (such as rosuvastatin, atorvastatin,
methotrexate) can result in increased concentrations (see section 5.2).
In vitro, encorafenib potentially inhibits a number of other transporters. Agents that are substrates of
renal transporters OAT1, OAT3, OCT2 (such as furosemide, penicillin) or agents that are substrates of
the hepatic transporters OCT1 (such as, bosentan) or substrates of P-gp (e.g. posaconazole) may also
have increased exposure.
Therefore, these agents, substrates of transporters should be co-administered with caution.
