• The mode of action suggests that the effects of L-dopa, dopaminergic agonists, and
anticholinergics may be enhanced by concomitant treatment with NMDA-antagonists such as
memantine. The effects of barbiturates and neuroleptics may be reduced. Concomitant
administration of memantine with the antispasmodic agents, dantrolene or baclofen, can modify
their effects and a dose adjustment may be necessary.
• Concomitant use of memantine and amantadine should be avoided, owing to the risk of
pharmacotoxic psychosis. Both compounds are chemically related NMDA-antagonists. The
same may be true for ketamine and dextromethorphan (see also section 4.4). There is one
published case report on a possible risk also for the combination of memantine and phenytoin.
• Other active substances such as cimetidine, ranitidine, procainamide, quinidine, quinine and
nicotine that use the same renal cationic transport system as amantadine may also possibly
interact with memantine leading to a potential risk of increased plasma levels.
• There may be a possibility of reduced serum level of hydrochlorothiazide (HCT) when
memantine is co-administered with HCT or any combination with HCT.
• In post-marketing experience isolated cases with international normalized ratio (INR) increases
have been reported in patients concomitantly treated with warfarin. Although no causal
relationship has been established, close monitoring of prothrombin time or INR is advisable for
patients concomitantly treated with oral anticoagulants.

Effect of parecoxib (or its active metabolite valdecoxib) on the pharmacokinetics of other medicinal
products
Treatment with valdecoxib (40 mg twice daily for 7 days) produced a 3-fold increase in plasma
concentrations of dextromethorphan (CYP2D6 substrate). Therefore, caution should be observed when
coadministering Dynastat and medicinal products that are predominantly metabolised by CYP2D6 and
which have narrow therapeutic margins (e.g. flecainide, propafenone, metoprolol).

A cocktail study was conducted in 16 healthy volunteers with twice-daily 500 mg tipranavir with
200 mg ritonavir capsule administration for 10 days to assess the net effect on the activity of hepatic
CYP 1A2 (caffeine), 2C9 (warfarin), 2D6 (dextromethorphan), both intestinal/hepatic CYP 3A4
(midazolam) and P-glycoprotein (P-gp) (digoxin). At steady state, there was a significant induction of
CYP 1A2 and a slight induction on CYP 2C9. Potent inhibition of CYP 2D6 and both hepatic and
intestinal CYP 3A4 activities were observed. P-gp activity is significantly inhibited after the first dose,
but there was a slight induction at steady state. Practical recommendations deriving from this study are
displayed below.

As stiripentol inhibited CYP2D6 in vitro at concentrations that are achieved clinically in plasma,
substances that are metabolized by this isoenzyme like: beta-blockers (propranolol, carvedilol,
timolol), antidepressants (fluoxetine, paroxetine, sertraline, imipramine, clomipramine), antipsychotics
(haloperidol), analgesics (codeine, dextromethorphan, tramadol) may be subject to metabolic
interactions with stiripentol. A dose-adjustment may be necessary for substances metabolised by
CYP2D6 and that are individually dose titrated.

Concomitant administration of sorafenib and midazolam, dextromethorphan or omeprazole, which are
substrates for cytochromes CYP3A4, CYP2D6 and CYP2C19 respectively, did not alter the exposure
of these agents. This indicates that sorafenib is neither an inhibitor nor an inducer of these cytochrome
P450 isoenzymes. Therefore, clinical pharmacokinetic interactions of sorafenib with substrates of
these enzymes are unlikely.

Methylnaltrexone bromide does not affect the pharmacokinetics of medicinal products metabolised by
cytochrome P450 (CYP) isozymes. Methylnaltrexone bromide is minimally metabolised by CYP
isozymes. In vitro metabolism studies suggest that methylnaltrexone bromide does not inhibit the
activity of CYP1A2, CYP2E1, CYP2B6, CYP2A6, CYP2C9, CYP2C19 or CYP3A4, while it is a
weak inhibitor of the metabolism of a model CYP2D6 substrate. In a clinical drug interaction study in
healthy adult male subjects, a subcutaneous dose of 0.3 mg/kg of methylnaltrexone bromide did not
significantly affect the metabolism of dextromethorphan, a CYP2D6 substrate.

In vitro studies with human liver microsomes showed that pazopanib inhibited CYP enzymes 1A2,
3A4, 2B6, 2C8, 2C9, 2C19, and 2E1. Potential induction of human CYP3A4 was demonstrated in an
in vitro human PXR assay. Clinical pharmacology studies, using pazopanib 800 mg once daily, have
demonstrated that pazopanib does not have a clinically relevant effect on the pharmacokinetics of
caffeine (CYP1A2 probe substrate), warfarin (CYP2C9 probe substrate), or omeprazole (CYP2C19
probe substrate) in cancer patients. Pazopanib resulted in an increase of approximately 30% in the
mean AUC and Cmax of midazolam (CYP3A4 probe substrate) and increases of 33% to 64% in the
ratio of dextrometrophan to dextrophan concentrations in the urine after oral administration of
dextromethorphan (CYP2D6 probe substrate). Co-administration of pazopanib 800 mg once daily and
paclitaxel 80 mg/m2 (CYP3A4 and CYP2C8 substrate) once weekly resulted in a mean increase of
26% and 31% in paclitaxel AUC and Cmax, respectively.

− Following co-administration of dextromethorphan and asenapine in healthy subjects, the ratio of
dextrorphan/dextromethorphan (DX/DM) as a marker of CYP2D6 activity was measured.
Indicative of CYP2D6 inhibition, treatment with asenapine 5 mg twice daily resulted in a
fractional decrease in DX/DM ratio to 0.43. In the same study, treatment with paroxetine 20 mg
daily decreased the DX/DM ratio to 0.032.
