HOS-110221 · Präparat HOS-093339
Substances that are inducers of CYP3A4 activity (e.g. dexamethasone, phenytoin, carbamazepine,
rifampicin, phenobarbital, fosphenytoin, primidone or Hypericum perforatum, also known as St.
John’s Wort) may significantly reduce exposure to Glivec, potentially increasing the risk of
therapeutic failure. Pretreatment with multiple doses of rifampicin 600 mg followed by a single
400 mg dose of Glivec resulted in decrease in Cmax and AUC(0-∞) by at least 54% and 74%, of the
respective values without rifampicin treatment. Similar results were observed in patients with
malignant gliomas treated with Glivec while taking enzyme-inducing anti-epileptic drugs (EIAEDs)
such as carbamazepine, oxcarbazepine and phenytoin. The plasma AUC for imatinib decreased by
73% compared to patients not on EIAEDs. Concomitant use of rifampicin or other strong CYP3A4
inducers and imatinib should be avoided.

Ulipristal acetate is metabolised by CYP3A4 in vitro.
- CYP3A4 inducers
In vivo results show that the administration of ulipristal acetate with a strong CYP3A4 inducer
such as rifampicin markedly decreases Cmax and AUC of ulipristal acetate by 90% or more and
decreases ulipristal acetate half-life by 2.2-fold corresponding to an approximately 10-fold
decrease of ulipristal acetate exposure. Concomitant use of ellaOne with CYP3A4 inducers (e.g.
barbiturates (including primidone and phenobarbital), phenytoin, fosphenytoin, carbamazepine,
oxcarbazepine, herbal medicines containing Hypericum perforatum (St. John’s wort),
rifampicin, rifabutin, griseofulvin, efavirenz and nevirapine) therefore reduces plasma
concentrations of ulipristal acetate and may result in a decreased efficacy of ellaOne. For
women who have used enzyme-inducing drugs in the past 4 weeks, ellaOne is not recommended
(see section 4.4) and non-hormonal emergency contraception (i.e. a copper intrauterine device
(Cu-IUD)) should be considered.

Hepatic metabolism: Interactions can occur with substances that induce CYP450 enzymes, resulting in
reduced concentrations of sex hormones and decreased effectiveness of combined oral contraceptives,
including Zoely. These substances are represented mostly with anticonvulsants (e.g. carbamazepine,
topiramate, phenytoin, phenobarbital, primidone, oxcarbazepine, felbamate); anti-infective drugs (e.g.
rifampicin, rifabutin, griseofulvin); St. John’s wort; bosentan and HIV or Hepatitis C virus (HCV)
protease inhibitors (e.g. ritonavir, boceprevir, telaprevir) and non-nucleoside reverse transcriptase
inhibitors (e.g. efavirenz).
Enzyme induction can occur after a few days of treatment. Maximal enzyme induction is generally

observed within a few weeks. After drug therapy is discontinued, enzyme induction can last for about
28 days.

Eviplera should not be co-administered with the following medicinal products as significant decreases
in rilpivirine plasma concentrations may occur (due to cytochrome P450 [CYP]3A enzyme induction
or gastric pH increase), which may result in loss of therapeutic effect of Eviplera:
• the anticonvulsants carbamazepine, oxcarbazepine, phenobarbital, phenytoin
• the antimycobacterials rifampicin, rifapentine
• proton pump inhibitors, such as omeprazole, esomeprazole, lansoprazole, pantoprazole,
rabeprazole
• the systemic glucocorticoid dexamethasone, except as a single dose treatment
• St. John’s wort (Hypericum perforatum)
