Since iloprost inhibits platelet function its use with the following substances may enhance iloprost-
mediated platelet inhibition, thereby increasing the risk of bleeding:
• anticoagulants, such as
- heparin,
- oral anticoagulants (either coumarin-type or direct),
• or other inhibitors of platelet aggregation, such as
- acetylsalicylic acid,
- non-steroidal anti-inflammatory medicinal products,
- non-selective phosphodiesterase inhibitors like pentoxifylline,
- selective phosphodiesterase 3 (PDE3) inhibitors like cilostazol or anagrelide,
- ticlopidine,
- clopidogrel,
- glycoprotein IIb/IIIa antagonists, like
o abciximab,
o eptifibatide,
o tirofiban,
- defibrotide.

Effects of anagrelide on other active substances
Anagrelide demonstrates some limited inhibitory activity towards CYP1A2 which may present a
theoretical potential for interaction with other co-administered medicinal products sharing that
clearance mechanism e.g., theophylline.
Anagrelide is an inhibitor of PDE III. The effects of medicinal products with similar properties
such as the inotropes milrinone, enoximone, amrinone, olprinone and cilostazol may be
exacerbated by anagrelide.

In vitro studies indicate that cladribine is a substrate of the equilibrative nucleoside (ENT1) and
concentrative nucleoside (CNT3) transport proteins. Accordingly, the bioavailability, intracellular
distribution and renal elimination of cladribine may theoretically be altered by potent ENT1 and CNT3
transporter inhibitors such as dilazep, nifedipine, nimodipine, cilostazol, sulindac or reserpine.
However, net effects in terms of potential cladribine exposure alterations are difficult to predict.
