Effects of other active substances on anagrelide
In vivo interaction studies in humans have demonstrated that digoxin and warfarin do not affect
the pharmacokinetic properties of anagrelide.
CYP1A2 inhibitors
Anagrelide is primarily metabolised by CYP1A2. It is known that CYP1A2 is inhibited by
several medicinal products, including fluvoxamine and enoxacin, and such medicinal products
could theoretically adversely influence the clearance of anagrelide.
CYP1A2 inducers
CYP1A2 inducers (such as omeprazole) could decrease the exposure of anagrelide (see
section 5.2). The consequences on the safety and efficacy profile of anagrelide are not established.
Therefore, clinical and biological monitoring is recommended in patients taking concomitant
CYP1A2 inducers. If needed, anagrelide dose adjustment could be made.

A major step in roflumilast metabolism is the N-oxidation of roflumilast to roflumilast N-oxide by
CYP3A4 and CYP1A2. Both roflumilast and roflumilast N-oxide have intrinsic phosphodiesterase-4
(PDE4) inhibitory activity. Therefore, following administration of roflumilast, the total PDE4
inhibition is considered to be the combined effect of both roflumilast and roflumilast N-oxide.
Interaction studies with CYP1A2/3A4 inhibitor enoxacin and the CYP1A2/2C19/3A4 inhibitors
cimetidine and fluvoxamine, resulted in increases of the total PDE4 inhibitory activity of 25%, 47%
and 59%, respectively. The tested dose of fluvoxamine was 50 mg. A combination of roflumilast with
these active substances might lead to an increase of exposure and persistent intolerability. In this case,
roflumilast treatment should be reassessed (see section 4.4).

Esbriet therapy due to the reduced clearance of pirfenidone. Other therapies that are inhibitors of both
CYP1A2 and one or more other CYP isoenzymes involved in the metabolism of pirfenidone
(e.g. CYP2C9, 2C19, and 2D6) should be avoided during pirfenidone treatment.
In vitro and in vivo extrapolations indicate that strong and selective inhibitors of CYP1A2 (e.g.
enoxacin) have the potential to increase the exposure to pirfenidone by approximately 2 to 4-fold. If
concomitant use of Esbriet with a strong and selective inhibitor of CYP1A2 cannot be avoided, the
dose of pirfenidone should be reduced to 801 mg daily (267 mg, three times a day). Patients should be
closely monitored for emergence of adverse reactions associated with Esbriet therapy. Discontinue
Esbriet if necessary (see sections 4.2 and 4.4).

Potential for other medicinal products to affect tasimelteon
CYP1A2 and CYP3A4 are enzymes identified to play a role in the metabolism of tasimelteon, with a minor
role for CYP2C9/C19. Medicinal products that inhibit CYP1A2 and CYP3A4 have been shown to alter the
metabolism of tasimelteon in vivo.
3
Strong CYP1A2 inhibitors (e.g. fluvoxamine, ciprofloxacin and enoxacin)
Caution should be used when administering tasimelteon in combination with fluvoxamine or other strong
CYP1A2 inhibitors such as ciprofloxacin and enoxacin because of a potentially large increase in tasimelteon
exposure and greater risk of adverse reactions: the AUC0-inf and Cmax of tasimelteon increased by 7-fold and
2-fold, respectively, when co-administered with fluvoxamine 50 mg (after 6 days of fluvoxamine 50 mg per
day). This is deemed even more important for strong CYP1A2 inhibitors also inhibiting other enzymes
involved in the clearance of tasimelteon (e.g. fluvoxamine and ciprofloxacin).
