Colchicine is a substrate of CYP3A4 and P-gp (an intestinal efflux transporter). Antibiotics Clarithromycin 500 mg Clarithromycin Cmax ↔ Whilst the changes in BID Clarithromycin AUC ↑ 19% clarithromycin parameters are not Clarithromycin Cmin ↑ 68% considered clinically relevant, the reduction in the 14-OH 14-OH-clarithromycin Cmax ↓ metabolite AUC should be 97% considered for the treatment of 14-OH-clarithromycin AUC ↓ infections caused by 97% Haemophilus influenzae in which 14-OH-clarithromycin Cmin ↓ the 14-OH metabolite is most 95% active. The increase of tipranavir Cmin may be clinically relevant. Tipranavir Cmax ↑ 40% Patients using clarithromycin at Tipranavir AUC ↑ 66% doses higher than 500 mg twice Tipranavir Cmin ↑ 100% daily should be carefully monitored for signs of toxicity of CYP 3A4 inhibition by clarithromycin and tipranavir. For tipranavir/r and P-gp (an patients with renal impairment intestinal efflux transporter) dose reduction of clarithromycin inhibition by clarithromycin. should be considered (see clarithromycin and ritonavir product information). Rifabutin 150 mg QD Rifabutin Cmax ↑ 70% Dosage reductions of rifabutin by Rifabutin AUC ↑ 190% at least 75% of the usual Rifabutin Cmin ↑ 114% 300 mg/day are recommended (ie 150 mg on alternate days, or 25-O-desacetylrifabutin Cmax ↑ three times per week). Patients 3.2 fold receiving rifabutin with Aptivus, 25-O-desacetylrifabutin AUC ↑ co-administered with low dose 21 fold ritonavir, should be closely 25-O-desacetylrifabutin Cmin ↑ monitored for emergence of 7.8 fold adverse events associated with rifabutin therapy. Further dosage Inhibition of CYP 3A4 by reduction may be necessary. tipranavir/r
